"Bipolar disorders are among the more common conditions affecting humankind and are, consequently, leading causes of disability worldwide. These illnesses are dynamic and complex, so that identifying them and then provide optimal management can be challenging. Because of this complexity, these conditions often comprise a disproportionate amount of most psychiatric practices, as specialty care that exceeds what can be delivered by other practitioners is often required. With these challenges in mind, we created this book to provide a comprehensive, readable review of these illnesses with chapters written by international experts in bipolar disorder"--

This new volume gathers important research developments dealing with bipolar disorders. In a world permeated with stress, bipolar disorders have become an all too common occurrence. There are millions of people throughout the world suffering from this disorder. Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in a person's mood, energy, and ability to function. Different from the normal ups and downs, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide. The primary psychological factor implicated in the manifestation of bipolar disorder is stressful life events. These can range from a death in the family to the loss of a job, from the birth of a child to a move. Once the disorder is triggered and progresses, 'it seems to develop a life of its own'. The two poles of bipolar disorder are mania and depression.

Although depression is the most common presentation of bipolar disorder, correct diagnosis generally requires a history of mania and thus presents a formidable challenge. This book provides clinicians with the necessary guidance to distinguish this illness and pursue an appropriate therapeutic course. It brings together a team of clinical investigators who offer cutting-edge research on the topic and address the most critical concerns regarding its treatment. Bipolar Depression first introduces a hierarchical model for diagnosis to allow the clinician to distinguish between bipolar and unipolar depression, addressing problems of misdiagnosis and overdiagnosis as well as differentiating attention-deficit/hyperactivity disorder (ADHD) and bipolar disorder. Early chapters review the neurobiology and genetics of this highly heritable condition, presenting studies of neurotransmitter function and brain imaging studies and documenting the susceptibility of specific chromosomes as loci for bipolar disorder. Other chapters address the particular issues of bipolar depression in children, for whom a diagnosis is especially problematic, and suicide, focusing on the need for assessment during both acute and maintenance treatment with interventions appropriate to a patient's symptoms and history. Bipolar Depression offers critiques of specific treatment approaches: Lithium and antiepileptic drugs: featuring a review of the most recent research on the use of lithium, in which higher doses are shown to be effective, plus coverage of lamotrigine, valproic acid, carbamazepine, oxcarbazepine, and topiramate. Antidepressants: offering new perspectives on a complex field, including a discussion of the randomized clinical trial literature and observational studies on their use, and citing cautions regarding side effects. Antipsychotics: evaluating the difference between first- and second-generation medications and discussing their role in controlling acute depressions. Novel approaches to treatment: including the use of atypical neuroleptics, electroconvulsive therapy, transcranial magnetic stimulation, vagus nerve stimulation, ketogenic diet, omega-3 fatty acids, myo-inositol, and dopamine agonists. Psychological interventions: focusing on the inclusion of cognitive-behavioral therapy or interpersonal social rhythm therapy for nonmelancholic depressions in patients who had previously received psychoeducation. Despite the past decade's advances in practice and research, there remains much room for progress in understanding and treating bipolar depression. This book blazes a trail toward that goal, opening new doors in recognizing differences between bipolar and unipolar forms of depression while offering both researchers and clinicians key insights into this troubling illness.

Background and Aims: Acting out in bipolar patients can be prepared and premeditated, but it can also be impulsive and brutal, in the form of drive discharge and of psychic economy . We aim to describe and analyse the socio-demographic , clinical characteristics and different impulsiveness scores of bipolar patients with cannabis use disorder .Methods: This is a descriptive cross-sectional and analytical study conducted with bipolar patients at Razi hospital. The patients recruited had a disorder of Cannabis use according to DSM-5 . Impulsivity was evaluated by the Barratt Impulsiveness Scale :BIS 11. (11)Results : We recruited 19 male patients . The average age of the patients was around 34, 11 years old .73,7% of patients were single and the most of them were of urban origin (84.2%) .A family history of mentel illness was found in 26.9% of the cases , 40,8 of which were bipolar disorders. 8 patients have attempted suicide (42,1%) .Thirteen out of nineteen patients had daily cannabis use . More than half of these patients consumed more than 4 joints a day and sixteen out of 19 patients (84.2%) had a high level of impulsivity . No significant relationship was found between impulsivity and cannabis use . Motor impulsivity has been correlated with age .Conclusion: Abuse of substance and especially cannabis, although it shares a high impulsivity with Bipolar Disorder, could not fully explain the elevation of the BIS 11 score in patients with this psychiatric disorder.

Although our knowledge of mood disorders is expanding, comparatively little is known about bipolar depression in particular. This book offers the most up-to-date information about the diagnosis, treatment, and research surrounding bipolar depression. Early chapters provide diagnostic information and review the course, outcome and genetics of this heritable condition. The book gives a thorough and unique overview of the neurobiology of the disorder, including neuroimaging work. Several chapters delineate the treatment of bipolar depression in special populations such as children and pregnant women. Furthermore, the particular issues of suicide, focusing on the need for assessment during both acute and maintenance treatment, are addressed. Finally, acute and long-term treatment strategies for bipolar depression are discussed, including both traditional and novel therapeutics, as well as non-pharmacological treatments. This volume offers researchers and clinicians key insights into this devastating disorder.

Bipolar disorders affect up to 3% of the world's population and are the 6th leading cause of disability worldwide. The estimated annual costs of bipolar disorder approach $50 billion in the United States, and the associated societal and personal suffering is immeasurable. Bipolar disorder is a lifelong illness that typically begins in mid-adolescence, thereby disrupting healthy developmental processes. It is also a progressive illness, making early intervention even more critical. As the disorder presents differently in youth as it does in adults, considerable controversy exists around the diagnosis in young people. The neurobiological factors leading to the onset of bipolar disorder reflect abnormalities in adolescent brain development. Bipolar disorder is strongly heritable and therefore, these developmental abnormalities likely represent the culmination of processes that precede the expression of symptoms. Current studies of children who are at risk for bipolar disorder are identifying these processes. Again, controversy exists about how to move these neuroscience gains forward to help affected individuals and, particularly to manage children at risk to delay or prevent the onset or progression of bipolar disorder. With these considerations in mind, Bipolar Disorder in Youth provides a timely, focused review of the diagnosis, treatment, and neurobiology of bipolar disorder in youth. It addresses current controversies and resolves those in which evidence is available. The book is organized into three sections based on these topics to provide comprehensive discussions to aid clinicians managing these individuals in their practices, as well as scientists trying to advance the field. The three editors are widely recognized experts in bipolar disorder, and the authors of each chapter represent international experts in the respective topics. Consequently, this book is the most comprehensive volume available discussing this important population and is a 'must' for the libraries of clinicians and scientists working with bipolar children and adolescents.

The revolution in psychiatry that began in earnest in the 1960s led to dramatic advances in the understanding and treatment of manic-depressive illness. Hailed as the most outstanding book in the biomedical sciences when it was originally published in 1990, Manic-Depressive Illness was the first to survey this massive body of evidence comprehensively and to assess its meaning for both clinician and scientist. It also vividly portrayed the experience of manic-depressive illness from the perspective of patients, their doctors, and researchers. Encompassing an understanding about the illness as Kraeplin conceived of it- about its cyclical course and about the essential unity of its bipolar and recurrent unipolar forms- the book has become the definitive work on the topic, revered by both specialists and nonspecialists alike. Now, in this magnificent second edition, Drs. Frederick Goodwin and Kay Redfield Jamison bring their unique contribution to mental health science into the 21st century. In collaboration with a team of other leading scientists, a collaboration designed to preserve the unified voice of the two authors, they exhaustively review the biological and genetic literature that has dominated the field in recent years and incorporate cutting-edge research conducted since publication of the first edition. They also update their surveys of psychological and epidemiological evidence, as well as that pertaining to diagnostic issues, course, and outcome, and they offer practical guidelines for differential diagnosis and clinical management. The medical treatment of manic and depressive episodes is described, strategies for preventing future episodes are given in detail, and psychotherapeutic issues common in this illness are considered. Special emphasis is given to fostering compliance with medication regimens and treating patients who abuse drugs and alcohol or who pose a risk of suicide. This book, unique in the way that it retains the distinct perspective of its authors while assuring the maximum in-depth coverage of a vastly expanded base of scientific knowledge, will be a valuable and necessary addition to the libraries of psychiatrists and other physicians, psychologists, clinical social workers, neuroscientists, pharmacologists, and the patients and families who live with manic-depressive illness.
