1. GENERAL The term "diagnostics" refers to the general theory of diagnosis, not to the study of specific diagnoses but to their general framework. It borrows from different sciences and from different philosophies. Traditionally, the general framework of diagnostics was not distinguished from the framework of medicine. It was not taught in special courses in any systematic way; it was not accorded special attention: students absorbed it intuitively. There is almost no comprehensive study of diagnostics. The instruction in diagnosis provided in medical schools is exclusively specific. Clinical instruction includes (in addition to vital background information, such as anatomy and physiology) specific instruction in nosology, the theory and classification of diseases, and this includes information on diagnoses and prognoses of diverse diseases. What is the cause of the neglect of diagnostics, and of its integrated teaching? The main cause may be the prevalence of the view of diagnostics as part-and parcel of nosology. In this book nosology is taken as a given, autonomous field of study, which invites almost no comments; we shall freely borrow from it a few important general theses and a few examples. We attempt to integrate here three studies: ll of the way nosology is used in the diagnostic process; of the diagnostic process as a branch of applied ethics; ~ of the diagnostic process as a branch of social science and social technology.

Independent duty hospital corpsmen assigned to submarines are normally the sole provider of medical care for the crew. Corpsmen with varying levels of experience and training must make diagnoses with limited diagnostic tools and without expert consultation. A medical evacuation (MEDEVAC) can potentially expose a submarine's position, is hazardous to both the patient and the rescuers, especially in high sea states, and is expensive because it entails the movement of large numbers of ships, aircraft and men. For these reasons, the Naval Submarine Medical Research Laboratory (NSMRL) was tasked in the 1970's with developing computerized diagnostic aids for these corpsmen with the goals of reducing the number of unnecessary medical evacuations and improving the quality of health care at sea. This report details the history and development of the computer assisted medical diagnostic aids at this laboratory and would be of benefit to individuals or Commands interested in continuing this type of work. Several of these programs have been approved for distribution by the Commanding Officer, NSMRL, and may be obtained by request. Amblyopia (lazy eye) affects about 2-5% of general population. It is a leading cause of single-eye blindness in adults. Video-based photoscreenirig, pioneered by Dr. Gerhard W. Cibis, is a technique effective for screening very young children for amblyopia-causing factors, which is essential to the prevention. This dissertation presents the first ever work to automate this screening technique. The main challenge in algorithm development in this study arises from many dynamic factors that can not be controlled, including the patient's direction of sight, focusing, and physical motion. These factors make the observed imagery highly variable even for the same patient. This dissertation provides detailed descriptions of the various algorithms that combine to reach screening decisions from the video data. The algorithms are divided into two main groups, image processing and computer-assisted diagnosis. Combining new image features and sequence processing, we have developed robust algorithms for locating irises, pupils, and Hirschberg points, which are useful for diagnosing misalignment between eyes. The new technical contributions include the use of sclera in iris boundary location and the use of possibilistic shell clustering with additional constraints to simultaneously locating a pair of pupils. We also investigated computer-assisted diagnosis of various amblyogenic vision disorders, including high refractive errors (hyperopia and myopia), refractive error difference between the two eyes (anisometropia), astigmatism, and misalignment between the two eyes, (strabismus). Diagnoses of these factors are then combined to yield the final screening decision. We evaluate the performance of our algorithm by comparing our screening decisions to those given by Dr. Cibis (the ground truth) over the same data set, which include video data of 182 different children. Our results indicate that the screening decisions made by the algorithms are 89% correct when compared with those made by Dr. Cibis. Further investigation points to several areas for future improvement, including better analysis of cases with misalignment and myopia.

Prostatic adenocarcinoma (CAP) is the second most common malignancy with an estimated 190,000 new cases in the USA in 2010 (Source: American Cancer Society), and is the most frequently diagnosed cancer among men. If CAP is caught early, men have a high, five-year survival rate. Unfortunately there is no standardized ima- based screening protocol for early detection of CAP (unlike for breast cancers). In the USA high levels of prostate-specific antigen (PSA) warrant a trans-rectal ultrasound (TRUS) biopsy to enable histologic confirmation of presence or absence of CAP. With recent rapid developments in multi-parametric radiological imaging te- niques (spectroscopy, dynamic contrast enhanced MR imaging, PET, RF ultrasound), some of these functional and metabolic imaging modalities are allowing for definition of high resolution, multi-modal signatures for prostate cancer in vivo. Distinct com- tational and technological challenges for multi-modal data registration and classifi- tion still remain in leveraging this multi-parametric data for directing therapy and optimizing biopsy. Additionally, with the recent advent of whole slide digital sc- ners, digitized histopathology has become amenable to computerized image analysis. While it is known that outcome of prostate cancer (prognosis) is highly correlated with Gleason grade, pathologists often have difficulty in distinguishing between interme- ate Gleason grades from histopathology. Development of computerized image analysis methods for automated Gleason grading and predicting outcome on histopathology have to confront the significant computational challenges associated with working these very large digitized images.

Computer technology has impacted the practice of medicine in dramatic ways. Imaging techniques provide noninvasive tools which alter the diag nostic process. Sophisticated monitoring equipment presents new levels of detail for both patient management and research. In most of these high technology applications, the computer is embedded in the device; its presence is transparent to the user. There is also a growing number of applications in which the health care provider directly interacts with a computer. In many cases, these appli cations are limited to administrative functions, e.g., office practice man agement, location of hospital patients, appointments, and scheduling. Nevertheless, there also are instances of patient care functions such as results reporting, decision support, surveillance, and reminders. This series, Computers and Medicine, will focus upon the direct use of information systems as it relates to the medical community. After twenty-five years of experimentation and experience, there are many tested applications which can be implemented economically using the current generation of computers. Moreover, the falling cost of computers suggests that there will be even more extensive use in the near future. Yet there is a gap between current practice and the state-of-the-art.

New imaging technology and more sophisticated image processing systems will have a profound effect on those areas of medicine which are concerned with imaging for diagnosis and therapy planning. Digitally formated data will form the basis of an increasing number of medical imaging modalities. Before the diagnostic imaging department of the future will largely be digital, many problems have still to be solved as regards image quality, costs, and ease of use. The computer and other information science derived methods will contribute towards solving many of the problems in these areas. It is widely expected that there will be an information science derived evolution in imaging for radiology and related departments. Computer assistance may be applied to image generation, e.g. CT, MRI, DR and DSR, storing and transferring of images, and viewing, analysing and interpreting of images. The application of computers to these activities (which characterise radiological departments), may be defined as Computer Assisted Radiology (CAR) . In the main, CAR will promote the transition from analog imaging systems to digital systems, integration of digital imaging modalities through Picture Archiving and Communication Systems (PACS') and the graduated employment of Medica~ Work Stations (MWS) for diagnosis and therapy planning. It will transfer geographically, organisationally and/or mentally isolate imaging activities towards fully integrated multi-imaging modality diagnostic departments. This development will have a considerable impact on patient management, on the medical profession and on the health care system.
