Gene-environment interactions are an area of increasing interest in complex human diseases. The first step in any study of the interactions between genes and the environment involves identifying genes which influence the trait of interest. In this dissertation, a new method for using the information in complex pedigrees to perform a joint linkage disequilibrium and linkage mapping of quantitative trait loci is developed. Subsequently, methods are needed to determine the interaction, if any, between these genes and environmental risk factors. Many of these factors, such as weight or age, are continuous and little is known about their distributions. Thus, we introduce a new method for estimating the gene-environment interaction parameters in a logistic regression for the case-control study design. In doing so, we make the assumption that in the underlying population, the distributions of the genetic factors and the environmental covariates are independent. Additionally, we treat the environmental parameters nonparametricly, utilizing the profile likelihood. Furthermore, the methodology we develop is also general enough to be used on many different types of genetic information, including haplotypes, and can accommodate missing genotype data. The method is also extended to allow analysis in the presence of population stratification or genotype misclassification. We show that the standard errors of parameter estimates using our method are smaller than those found using complete data only. These methods are illustrated using simulations and are applied to a real data set exploring the interaction between genotype and environment in disease risk.

Most diseases are not based solely on genetic factors, but are instead a mix of genetic and environmental factors. Genetic association studies have become a standard approach to identifying genetic risk factors for disease and examining how those genetic risk factors interact with environmental factors to influence risk of disease. In some genetic association studies, the cases are collected with detailed clinical and demographic information, but due to costs of genotyping, the controls are drawn from an existing data base. There are multiple ways to potentially test for a gene-environment interaction when not much is known about the controls, including case-only analysis, stratified analysis, case-parental control study, and affected relative pair study. This thesis examined the best alternative analysis approach, in terms of power, for gene-environment interactions in a case-control study when there are many more controls than cases, and when there is no information on the environmental factors among controls. For a case-control study with unrelated cases and controls, three alternative potential solutions to the problem of having no environmental information on the controls were compared: the case-only to two modified stratified analyses. Simulation studies were carried out in addition to a real data analysis. The case-only analysis was the most powerful approach for testing for a gene-environment interaction when the information on the environment is missing for the controls, however the power for the stratified analyses was not greatly reduced compared to the case-only approach for most of the simulation conditions we considered. For a genome-wide analysis, one recommended approach based on this work is to use both the case-only and stratified approaches with cautious interpretation of strong interaction results that exist only for the case-only approach.

Our DNA, 3 billion letters long, is the blueprint of life. People differ from each other in around 1% of these letters. However, it is largely unknown which of these differences cause disease, nor how exactly changes in DNA ultimately lead to disease. In my PhD research I contributed to the detection of these genetic risk factors and studied how these DNA changes actually disrupt the correct functioning of cells. Using the "Genome of the Netherlands" project results, in which the BBMRI-NL consortium fully mapped the DNA of 250 Dutch families, I was able to improve genetic research in other groups of people and thereby better detect genetic risk factors. I then looked at how these DNA changes disrupt individual cells in a large number of people participating in various Dutch bio-banks such as Lifelines. By looking at DNA, RNA, epigenetics and proteins at the same time, I was able to discover many genetic risk factors, which biological processes they disrupt, and which ones offer starting points for the development of new medicines that can repair these disrupted processes. In addition, through the large-scale re-use of RNA data, I was able to map how many genes relate together and to disease. With these big data analyses, I subsequently developed a new algorithm that is now being used to make a diagnosis more quickly in people with a serious illness. With my PhD research, I contributed to the immediate improvement of patient care and collected new knowledge that could help in the longer term with the development of new medicines.
