Advances in long-term improvement and outcomes of patients with kidney disease will require the use of novel biomarkers to identify patients at high risk for kidney disease and to diagnose kidney disease early for effective treatment. A biomarker is a substance found in the blood, body fluids or tissues that provides a measure of normal biological or pathological processes or response to pharmacological compounds or drugs. There are a wide variety of biomarkers including but not limited to mRNA, proteins and peptides, and lipid molecules. In AKI, important pathophysiological processes such as inflammation, apoptotic and necrotic cell death and, tubule regeneration may be reflected in blood or urine. An array of candidate markers along with clinical information in long-term clinical studies with appropriate analytical methodologies will likely provide prognostic information. Despite well-known limitations, currently the most widely used biomarkers for the early diagnosis of CKD and AKI are proteinuria, serum creatinine and blood urea nitrogen. Most clinicians are aware that serum creatinine and blood urea nitrogen are poor biomarkers due to inherent characteristics of these molecules and handling by the kidney. Creatinine is secreted and urea nitrogen is reabsorbed by the renal tubules. Many endogenous substances interfere in the assay for creatinine. Serum creatinine and urea appear late after acute kidney injury and the serum levels in part depend on the generation (large or small body mass). Acute kidney injury is a non steady state condition thus serum creatinine and urea nitrogen will lag behind kidney injury. For these reasons new biomarkers are imperative. With knowledge of these limitations in use of current biomarkers and the lack of progress in reducing the mortality and morbidity from kidney disease, there has been a great surge of interest in identifying novel biomarkers with a particular emphasis on the early diagnosis of kidney disease. A variety of methods have been employed including transcriptomics, proteomics, gene arrays and lipidomics. Currently, candidate biomarkers have been found in different disorders and have been tested in humans and many candidate biomarkers have yet to be identified. Most studies to date are preliminary and require validation in large multicentre studies followed by commercial assay development validation and testing. This new book outlines the rapid advances made in the field of biomarker development for kidney disease in which a variety of novel molecules have been identified and studied in humans.

Abstract : Purpose of review: Chronic kidney disease (CKD) is a well-known risk factor for cardiovascular disease (CVD). Renal biomarkers might be valuable in predicting CVD. Investigation of these biomarkers may uncover some of the poorly understood mechanisms that link renal and CVD as well as aid in the modification of disease and serve as a useful tool in diagnosing early disease and monitoring therapeutic responses. In this review we discuss the clinical utility of emerging and known renal biomarkers in predicting CVD. Recent findings: Prior to adopting a biomarker into routine clinical practice, evidence-based laboratory medicine requires optimal technical and analytical performance, which is a prerequisite to have confidence in the result. Furthermore, an ideal biomarker should have evidence of its utility in predicting clinical, therapeutic and other health outcomes as well as proving its organizational impact and cost-effectiveness. The renal biomarkers that have been associated with CVD include cystatin C as a better marker of glomerular filtration than creatinine, albuminuria, neutrophil gelatinase associated lipocalin, a marker of acute kidney injury, fibroblast growth factor-23 and parathyroid hormone. Only urine albumin has been adopted into routine clinical practice. Summary: Of all the renal biomarkers, only albumin is clearly associated with CVD. The other biomarkers are earlier in clinical development and the evidence base for their clinical utility needs to be expanded substantially before they can be adopted into routine practice.

Abstract : Supplemental Digital Content is available in the text Abstract : Abstract: Acute kidney injury (AKI) is associated with increased morbidity and mortality and is frequently encountered in coronary care units (CCUs). Its clinical presentation differs considerably from that of prerenal or intrinsic AKI. We used the biomarkers calprotectin and neutrophil gelatinase-associated lipocalin (NGAL) and compared their utility in predicting and differentiating intrinsic AKI. This was a prospective observational study conducted in a CCU of a tertiary care university hospital. Patients who exhibited any comorbidity and a kidney stressor were enrolled. Urinary samples of the enrolled patients collected between September 2012 and August 2013 were tested for calprotectin and NGAL. The definition of AKI was based on Kidney Disease Improving Global Outcomes classification. All prospective demographic, clinical, and laboratory data were evaluated as predictors of AKI. A total of 147 adult patients with a mean age of 67 years were investigated. AKI was diagnosed in 71 (50.3%) patients, whereas intrinsic AKI was diagnosed in 43 (60.5%) of them. Multivariate logistic regression analysis revealed urinary calprotectin and serum albumin as independent risk factors for intrinsic AKI. For predicting intrinsic AKI, both urinary NGAL and calprotectin displayed excellent areas under the receiver operating characteristic curve (AUROC) (0.918 and 0.946, respectively). A combination of these markers revealed an AUROC of 0.946. Our result revealed that calprotectin and NGAL had considerable discriminative powers for predicting intrinsic AKI in CCU patients. Accordingly, careful inspection for medication, choice of therapy, and early intervention in patients exhibiting increased biomarker levels might improve the outcomes of kidney injury.

Abstract : Purpose of review: Although recovery of kidney function following acute kidney injury (AKI) is not uncommon, it is often incomplete and associated with the development of chronic kidney disease (CKD). In order to improve AKI management, there is a critical need to develop a series of tests and biomarkers to detect renal function recovery and identify patients with progressive kidney disease. This article examines the current body of literature in the field. Recent findings: The recently established consensus definition for AKI has resulted in significant advances in pathophysiologic understanding, patient identification, and disease prognostication. Unfortunately, the definition for renal recovery following AKI remains inconsistent. Proteinuria and microalbuminuria – classical markers of CKD progression – have been used and validated for the progression of AKI to CKD. Data on the performance of other biomarkers of kidney repair and the progression toward CKD are very limited. Specifically, the role of novel biomarkers including neutrophil gelatinase-associated lipocalin, kidney injury molecule-1, and nephronectin (NPNT) in the recovery process has been studied, but it has not reached the point of widespread clinical implementation. Summary: There is a critical need for translational and clinical investigations to verify the performance of potential kidney injury repair and progression biomarker candidates in the clinical setting.

Background and goal of study: Acute kidney injury (AKI) is a frequent complication after orthotopic liver transplantation (OLT) and it`s early detection is of crucial importance for an appropriate management. Since the traditional kidney function parameters like serum-creatinine (s-Cr) have severe limitations concerning early detection of AKI, novel biomarkers such as TIMP-2 and IGFBP7, have been studied and shown to be superior in previous studies in critically ill patients. The aim of our study was to evaluate the predictive value of these novel biomarkers during the early perioperative period after human OLT. Materials and Methods: Paired urine and blood samples from patients undergoing OLT were collected at five different time points (TP): after induction of anesthesia (TP0), end of surgery (TP1) and at day 1 and 2 after OLT (TP2, TP3). The incidence of perioperative AKI was assessed according to the Acute Kidney Injury Network (AKIN) criteria. The detection of urinary TIMP-2 and IGFBP7 was performed using the NephroCheck (NC) test (Astute140u00aeMeter), an ELISA based assay, which defines the risk score for AKI. Data are presented as mean u00b1 standard deviation (SD). Results and Discussion: Forty liver transplant recipients were enrolled in this study. Sixteen (40%) patients developed AKI stage 2 or 3 within 48 h post OLT and 6 (15%) patients were supported with renal replacement therapy. There was no significant difference in the receiver operating characteristics curve analysis at TP0 and TP1 between the NC test and s-Cr in regard of development of AKI 2 or 3 within the first 48 hours post OLT. S-Cr at TP2 performed even better (AUC 0.83; CI 0.68-0.93; p=0.0001) than the NC test (AUC 0.51; CI 0.34-0.67; p=0.26). Only on day 2, both, s-Cr (AUC 0.90; CI 0.76-0.97; p=0.0001) and NC (AUC 0.70; CI 0.54-0.84; p=0.02) predicted AKI 2 and 3. Conclusions: TIMP-2 and IGFBP7 are not superior to the traditional kidney function parameters in predicting development of perioperative AKI II and III in clinical setting of human OLT.
