This book is the updated English version (not a mere translation) of a textbook which appeared successively in Czech,in German, in Bulgarian, in Dutch and in Polish. This English version is shorter, more concise and includes the latest techniques. The role of manipulation is limited to passive mobility and an important part of this book deals with active mobility. The author emphasizes that this is a textbook and not a manual of techniques: the latter deals with individual techniques, while his purpose is to show that it can be disastrous to confine one's interest in this manner and to remain unaware of both the broader context of treatment and of the possible alternatives. There are chapters on functional anatomy, the diagnosis and treatment of disturbed locomotor system function, including indications of appropriate treatment and the place of manipulation, and finally a long chapter on the clinical aspects of impaired function of the locomotor system. The book begins by discussing the major role played by impaired function in the vast majority of patients suffering from pain arising in the locomotor system. This type of pain is altogether the most frequent from which patients suffer, and the first two chapters of this book are mainly concerned with the origin of this pain. It is traditionally associated with rheumatism, but unfortunately rheumatologists are insufficiently aware of locomotor function and leave this field largely to neurologists and orthopaedic surgeons. The author believes that the speciality which is principally concerned with impaired function and its restoration to normal is rehabiliation medicine, a term which includes physical therapy, and it is in this framework that the future of manipulation lies. This will continue to be the case until, one day, a new speciality is established, one which deals with the whole of the locomotor system and in particular with its function. Ideally, it should be called "Musculo-skeletal Medicine".

It is fashionable in professional circles to deplore the difficulty of intellectual discourse between "academicians" - men of letters, researchers, rationalist- and "practitioners" - surgeons, radiologists, physical therapists. Even this suspected gap between "two cultures" came very near together in the wonderful town of Groningen (The Netherlands) as wise men from both practice and scholarship gathered to talk about the unfathomables of the temporomandibular joint. There were keen discussions about the intense bio logical changes which occur about the complex temporomandibular joint after excessive use or injury. These papers were followed by talks outlining the experiences of those involved in the imaging and non-surgical and surgical management of patients who were enduring such changes. The pitch and interchange of opinions and evidence as to why a disc or its position could effect little or profound disturbance of the temporomandibular apparatus were en lightening to each who listened - and thought. And even more sobering was to hear the report of a well-documented, multiple decades long study of a large number of patients with osteoarthrosis and internal derangement which defined a natural course and eventual end of the disease. With this understanding, one is now faced with the obvious question of how much treatment patients with osteoarthrosis really require.

Phagocytosis has been at the forefront of cell biology for more than a century. Initially, phagocytosis, which comes from Greek words meaning “devouring cells,” was discovered in the late 19th century by Ilya Metchnikoff, who was awarded, together with Paul Ehrlich, the Nobel Prize in Physiology and Medicine in 1908 “in recognition of their work on immunity.” At that time Metchnikoff had already identified a function for phagocytes not only in host defense but also as scavengers of degenerating host cells during metamorphosis of tadpoles, thus providing one of the first descriptions of apoptotic cell clearance by macrophages (Kaufmann 2008). Since then, much has been learned about phagocytosis, and the previous several decades have witnessed outstanding progress in understanding the functions and the molecular mechanisms of phagocytosis. Two main types of targets are cleared by phagocytosis: microbial pathogens and dying cells. Rapid recognition and clearance of dying cells by phagocytes plays a pivotal role in development, maintenance of tissue homeostasis, control of immune responses, and resolution of inflammation. Clearance of dying cells can be divided into several stages, including sensing, r- ognition, binding and signaling, internalization, and immunological responses. In this book, our contributors address these different stages of dead cell cle- ance and examine how impaired clearance of dying cells may lead to human d- eases. We have attempted to provide sufficient cross-referencing and indexing to enable the reader to easily locate the ideas elaborated in the different chapters.
