Obesity has devastating effects on a patient's overall health, with specific negative effects on organ systems, long-term. The hepatologist must often manage diseases of the liver and pancreas as a result of obesity. This issue will provide a current update on the diagnosis, treatment, and management of NAFLD and NASH. Dr. Bernstein has assembled the top leaders in the field to provide timely clinical reviews. Articles are devoted to the following topics: Clinical and economic burden of NAFLD/NASH; NAFLD/NASH in children and its implications; Natural history of NAFLD/NASH; Diagnosis and evaluation of NAFLD/NASH; Radiological imaging in NAFLD/NASH; The use of liver biopsy in NAFLD: When to biopsy and in whom; Pathophysiology of NAFLD/NASH; Risk factors for the development of NAFLD/NASH including genetics; Role of intestinal microbes in NAFLD/NASH; NAFLD/NASH and the metabolic syndrome; NAFLD/NASH and lipid and insulin resistance; NAFLD/NASH and cardiac disease; Current treatment of NAFLD/NASH; Emerging treatment of NAFLD/NASH; NAFLD/NASH and HCC and NAFLD/NASH and liver transplantation. Readers will have a clear understanding of how to manage outcomes for these patients.

The Hepatic circulation is unique among vascular beds. The most obvious unique features include the dual vascular supply; the mechanism of intrinsic regulation of the hepatic artery (the hepatic arterial buffer response); the fact that portal blood flow, supplying two thirds of liver blood flow, is not controlled directly by the liver; the fact that 20% of the cardiac output rushes through the most vascularized organ in the body, driven by a pressure gradient of only a few millimeters of mercury; the extremely distensible capacitance and venous resistance sites; the unidirectional acinar blood flow that regulates parenchymal cell metabolic specialization; and the high concentration of macrophagic (Kupffer) cells filtering the blood. The liver is the only organ reported to have regional blood flow monitored by the autonomic nervous system. This mechanism, when dysfunctional, accounts for the hepatorenal syndrome and offers a mechanistic therapeutic target to treat this syndrome. The trigger for liver regeneration is dependent on hepatic hemodynamics so that chronic liver blood flow regulates liver cell mass. In severe liver disease, the whole body circulation is reorganized, by forming portacaval shunts, to accommodate the increased intrahepatic venous resistance. These shunts protect the venous drainage of the splanchnic organs but lead to loss of major regulatory roles of the liver. The development of knowledge of the hepatic vasculature is presented from a historical perspective with modern concepts summarized based on the perspective of the author's four decades of devotion to this most marvelous of organs. Hardbound. Since the first Congress on Free Radicals in Liver Injury took place in 1985, much research and development has taken place in this area of liver pathophysiology which is now considered to be of prime interest and importance in hepatology. In these Proceedings of the second Congress, held in Turin in June 1988, the most recent advances in the study of free radical mechanisms, from the organic to the molecular level in both acute and chronic liver injuries are discussed, in both experimental animals and man. The first section covers the remarkable advances that have been made in the clarification of biochemical mechanisms connecting oxidative insult to hepatocyte death. Stimulating new data on the role of free radicals and lipid peroxidation in the development of hepatocellular carcinoma and in liver regeneration appear in the second section. The papers in the third section investigate the involvement of oxidative species in the pathology of chr
