The measurement of serum levels of etoposide using high performance liquid chromatography was investigated in patients with small cell carcinoma of the lung. 19 patients were treated with two cycles of etoposide (600 mg/m2 daily) on three days as a six hour infusion, 2 patients were received modified schedule with the same dose given as a continuous 72 hour infusion during the second cycle. Peak levels of etoposide ranged between 17.5 ug/ml and 66.0 pg/ml (mean 43.0 ug/ml) during cycle 1 and between 22.5/ml and 66.0 ug/ml (mean 45ug/ml) for cycle 2. Variation was seen in the area under the concentration curve of the drug (AUC) with the range for the course 1 being 698 to 1799 g/ml/h and for cycle 2 1054 to 1803 pg/ml/h. The AUC for the second cycle of treatment in the 2 patients who received the drug as a continuous infusion were 44% and 66% that obtained during the first cycle of treatment. The median survival of the patients in the trial was 262 days with a single patient surviving over 1500 days. No correlation was found between dose of drug, plasma levels of etoposide or AUC values with survival or response to treatment. Severe dose-limiting toxicity (neutropenic septicaemia) was seen in all patients and was most severe following the first treatment. No correlation was seen with toxicity and plasma levels or dose of etoposide. The measurment of drugs used in the treatment of cancer is being increasingly advocated, however, measurement in plasma may prove unsatisfactory since it may not reflect drug levels or activity at the target site.

Lung cancer is primarily a disease of well-developed affluent countries. It is by far the most common cancer in men, and is poised to overtake breast cancer as the most common cancer in women. Yet, although it is common, preventable, and sometimes curable, very little information isavailable about what happens when you get it - which is when accessible information is most needed. In a clear, non-emotive style, this revised and updated new edition provides the answers to the questions patients with lung cancers or those involved with them are likely to ask. The first section describes the causes (and includes a chapter on stopping smoking), diagnosis, and progression of thedisease. There follows a section on the curative and symptomatic treatments available for the different types of lung cancer; and the third section, new for this edition, discusses the future prospects for the disease and its treatment. Also included are a glossary, a list of further reading, andan extensive list of addresses of organizations which can provide help and advice.

Lung Cancer: Over the course of the last decade, the treatment of lung cancer has evolved quite rapidly. New scientific and clinical advances have modified the standard of care and led to improved patient outcomes. At the same time, the treatment of lung cancer has become increasingly complex, requiring the comprehensive review and assessment of multiple issues, genetics, radiology, surgery, reconstruction, chemotherapy, and more. As a result the harmony and open communication between these specialties facilitated by a multidisciplinary team approach are crucial in providing the best care to patients and ensuring successful treatment. Lung Cancer: A Multidisciplinary Approach to Diagnosis and Management, written by a multidisciplinary team of authors representing a range of disciplines, is a valuable resource for physicians, fellows, nurses, physician assistants, physical therapists, and all health care providers involved in the treatment of lung cancer. Lung Cancer: A Multidisciplinary Approach to Diagnosis and Management summarizes the state-of-the-art issues related to the treatment of lung cancer and describes an approach for optimal multidisciplinary care for individuals who have been diagnosed with lung cancer or who are at higher risk to develop lung cancer. About the Series: The Current Multidisciplinary Oncology series edited by Charles R. Thomas consolidates and integrates the varied aspects of multidisciplinary care for major topics in oncology, including breast, lung, prostrate, head and neck and more. The volumes in the Current Multidisciplinary Oncology series will represent all related topic areas, including oncology, radiation oncology, pain, pathology, imaging, psychological support and the primary disease. In addition, each volume includes a chapter focusing on special populations and the disease's impact / difference on them, and discussion of future directions and quality of life issues. In addition each volume has a chapter written by a private practice oncologist. All Current Multidisciplinary Oncology titles provide: Consolidation and integration of the varied aspects of multidisciplinary care for major topics in oncology Coverage of all related topic areas, including medical and surgical oncology, radiation oncology, pain, pathology, imaging, psychological support, and the primary disease A chapter focusing on special populations and the disease's impact / difference on them A chapter on community practice written by a private practice oncologist Discussion of quality-of-life issues

Defining the Lung Cancer Problem 1 Lung cancer is the leading cause of cancer death in the world. It kills almost as many Americans as cancers of the breast, prostate, colon, rectum, pancreas, and 2 kidney combined, and accounts for 28.6% of all US cancer deaths. With an increase in the 5-year relative survival rate from 13% to only 16% in the more than 2 30 years from 1974 to the present, it will take us another 840 years to eradicate lung cancer deaths if we do not improve the current rate of progress. As discussed in this text, lung cancer prevention has received substantial att- tion. The decrease in smoking in recent decades has helped, but smoking is not the only problem. Lung cancer in people who have never smoked is currently the 5th 3 leading cause of cancer death in the United States. Several factors contribute to the lethality of lung cancer, including the rapidity of tumor growth, advanced stage at diagnosis (due to nonspecificity of early sy- toms and the uncertain efficacy of screening), early development of metastases, and resistance to therapy. Several chapters in this book discuss new molecular targets that may be potentially exploitable in the future, as well as discussing our track record to date in exploiting them.

Leading physician scientists and noted researchers review novel methods for determining the etiology of a variety of lung cancers and present readily reproducible techniques for examining the associated multitude of genetic abnormalities. The methods make it possible to detect these alterations at the cellular, DNA, and protein levels, to study the development of lung cancer in vitro and in vivo-either in situ or in the form of metastases, and to test targeted therapies with detailed model systems. An animal models section gives explicit instructions for setting up and testing these systems, which are rarely described in the literature.
