Background: The Artificial Pancreas (AP) project has a longstanding evolution worldwide1, but until this moment was never tried in Latin America where the diet and lifestyle is somehow different. Patients with type 1 diabetes mellitus (T1DM) treated with Continuous Subcutaneous Insulin Infusion (CSII) and Continuous Glucose Monitoring (CGM) still need much training and medical assistance to achieve a safe glucose profile. An AP consists of a CSII and a CGM connected by a control algorithm that aims to regulate the insulin infusion rate to maintain glycemic values in range (70- 180 mg/dl) by creating a closed loop (CL) system. The main purpose of the AP system is to maintain acceptable mean glycemic ranges without increasing the number of hypoglycemic episodes, even during challenging periods of the day such as meals, exercise and overnight.Aims: To perform the first ever AP pilot study in Latin America in which the primary outcome measure is to check adequate operation of the system and to train a team of professionals in Argentina on the use of DiAs (UVA Diabetes Assistance) containing the USS algorithm. The system is considered verified by proper operation for more than 80% of the total time and maintenance of the glycemic profile within desirable glucose range (70-180 mg/dl) or acceptable glucose range (181-250 mg/dl). Reduction of hypoglycemic events is evaluated as a secondary outcome. Furthermore, an additional aim of this study is to prepare the team for future clinical trials using a new algorithm (ARG: Automatic Regulation of Glucose) developed in Argentina by ITBA, UNQ and UNLP and adapted to be used in DiAs.Method: This is a non-randomized pilot study of five patients mean age 43,4u00b17,4 years old with T1DM mean 17u00b14,0 years of diagnosis and mean HbA1c 7,84u00b10,4%. After 36 hours run-in training period of sensor-augmented pump (SAP) at home the patients were admitted for 36 hours at the Hospital Italiano of Buenos Aires where they were fitted with the AP along with UVA algorithm (USS: unified safety system) programmed in the DiAs platform. Regulatory approval has been obtained as required by local laws.Results: All 5 patients remained in closed loop (CL) in adequate operation for >94.29% of total time. During 36 hours the mean percentages of time in CL with glycaemia between 70-250 mg/dl was 97.4%; between 70-180 mg/dl it was 79.5%; and 70 mg/dl it was 1.6%, all with an IC of 95%. Overnight (23 to 7 hours) the mean percentages of time in CL with glycaemia between 70-250 mg/dl was 100%; between 70-180 mg/dl it was 87%; and

This brief introduces recursive modeling techniques that take account of variations in blood glucose concentration within and between individuals. It describes their use in developing multivariable models in early-warning systems for hypo- and hyperglycemia; these models are more accurate than those solely reliant on glucose and insulin concentrations because they can accommodate other relevant influences like physical activity, stress and sleep. Such factors also contribute to the accuracy of the adaptive control systems present in the artificial pancreas which is the focus of the brief, as their presence is indicated before they have an apparent effect on the glucose concentration and so can be more easily compensated. The adaptive controller is based on generalized predictive control techniques and also includes rules for changing controller parameters or structure based on the values of physiological variables. Simulation studies and clinical studies are reported to illustrate the performance of the techniques presented.

Diabetes mellitus is a widespread chronic disease in the modern world that affects millions of people around the globe. In Singapore, one in ten of the population has diabetes, and the severity of the problem has prompted the country’s prime minister to talk about the disease at the National Day Rally in 2017. Designing an artificial pancreas that can provide effective blood glucose control for individuals with diabetes is one of the most challenging engineering problems. The author reports on research into the development of an implantable artificial pancreas that can regulate blood glucose levels by delivering appropriate dosages of insulin when necessary. By sensing blood glucose and injecting insulin directly into the vein, the implantable device aims to remove delays that occur with subcutaneous blood glucose sensing and insulin delivery. Preliminary in-vitro and in-vivo experimental results suggest that the implantable device for blood glucose control could be a clinically viable alternative to pancreas transplant.

Abstract: Technological advances have made the artificial pancreas a reality. This has the potential to improve the lives of individuals with Type 1 diabetes by reducing the risk of hypoglycaemia, achieving better overall glucose control, and enhancing quality of life. Both single‐hormone (insulin‐only) and dual‐hormone (insulin and glucagon) systems have been developed; however, a focused review of the relative benefits of each artificial pancreas system is needed. We reviewed studies that directly compared single‐ and dual‐hormone systems to evaluate the efficacy of each system for preventing hypoglycaemia and maintaining glycaemic control, as well as their utility in specific situations including during exercise, overnight and during the prandial period. We observed additional benefits with the dual‐hormone artificial pancreas for reducing the risk of hypoglycaemic events overall and during exercise over the study duration. The single‐hormone artificial pancreas was sufficient for maintenance of euglycaemia in the overnight period and for preventing late‐onset post‐exercise hypoglycaemia. Future comparative studies of longer duration are required to determine whether one system is superior for improving mean glucose control, eliminating severe hypoglycaemia, or improving quality of life. What's new?: We review the literature comparing the relative efficacy and utility of single‐ and dual‐hormone artificial pancreas systems. The pharmacokinetic profile of glucagon, with rapid onset and short duration of action, confers the major benefit of the dual‐hormone artificial pancreas. The dual‐hormone artificial pancreas is superior to single‐hormone systems in reducing hypoglycaemia overall and during exercise in short‐term studies, whereas there is no strong evidence of added benefit for preventing nocturnal or post‐exercise hypoglycaemia. The relative benefit of each system over the long term for improving HbA1c, preventing severe hypoglycaemia, and affecting quality of life remains unknown.

The increase in the availability and reliability of network connections lets envision systems supporting a continuous remote monitoring of clinical parameters useful either for overseeing chronic diseases or for following clinical trials involving outpatients. We report here the results achieved by a telemedicine infrastructure that has been linked to an artificial pancreas platform and used during a trial of the AP@home project, funded by the European Union. The telemedicine infrastructure is based on a multiagent paradigm and is able to deliver to the clinic any information concerning the patient status and the operation of the artificial pancreas. A web application has also been developed, so that the clinic staff and the researchers involved in the design of the blood glucose control algorithms are able to follow the ongoing experiments. Albeit the duration of the experiments in the trial discussed in the article was limited to only 2 days, the system proved to be successful for monitoring patients, in particular overnight when the patients are sleeping. Based on that outcome we can conclude that the infrastructure is suitable for the purpose of accomplishing an intelligent monitoring of an artificial pancreas either during longer trials or whenever that system will be used as a routine treatment.
