In the last quarter century, advances in mass spectrometry (MS) have been at the forefront of efforts to map complex biological systems including the human metabolome, proteome, and microbiome. All of these developments have allowed MS to become a well-established molecular level technology for microorganism characterization. MALDI-TOF-MS has the ability to streamline identification of microorganisms in the microbiology lab. In this project, we focused on two main aims. First, the implementation of this technology for direct blood culture identification. Within this aim we determined the limit of detection for the protein purification method we planned on using in our laboratory, determined handling conditions for the samples, and evaluated the MALDI-TOF-MS for concordance with our current workflow and reports. Second, customization of the commercial library for Cryptococcus gattii was performed. Within this aim, we improved representation of the species in the MALDI-TOF-MS library, and also explored ways to determine the subspecies based on gathered spectra.

"Colorectal cancer (CRC) is known to be one of the most aggressive types of cancer.While the gut is a diverse ecological inhabitant of thousands of bacterial species, rising evidencelinks Fusobacterium nucleatum (F. nucleatum) with the disease. Whether this bacterium is acausative factor for the disease or a species that merely happens to flourish in the CRCenvironment, is not fully determined. F. nucleatum has been further classified into fourgenetically different subspecies; nucleatum, polymorphum, animalis, and vincentii. Thesesubspecies are thought to vary in their virulence and the various diseases they might cause.Up to that time, conventional matrix-assisted laser desorption ionization- time-of-flightmass spectrometry (MALDI-TOF MS) has been used as a proteomics tool to detect the F.nucleatum four subspecies, in pure cultures. Recently, the more advanced tandem time-offlight MALDI-MS (MALDI-TOF/TOF MS) technology has been introduced as an efficient andaccurate method of detecting bacterial pathogens at microbiology clinical diagnosticlaboratories.Human saliva has been proposed as a potential route of F. nucleatum strains to reachand colonize the CRC tissue. Along with the non-invasiveness, time-effectiveness, and ease ofacquisition, saliva represents a potential mirror for a myriad of systemic health changes inhumans. Hence, human saliva has been extensively used as a promising diagnostic tool inhealthcare. For this master’s thesis, we explored the capability of MALDI-TOF/TOF MS toidentify F. nucleatum to the subspecies level in saliva samples. The analyzed saliva specimens11were obtained from patients with precancerous adenoma, the primary precursor for CRC, andfrom non-diseased controls.Results: We have optimized a proteomics approach that allows for the successfulidentification of F. nucleatum at the subspecies-level directly from saliva samples. Our findingshighlight the presence of the F. nucleatum subspecies in both colorectal adenoma patients'saliva and non-diseased controls’ saliva. We have also shown a difference in the F. nucleatumsubtypes inhabiting the diseased and non-diseased subjects. Specifically, F. nucleatum subsp.nucleatum was absent in the patients’ saliva while it was detected in the non-diseased controlgroup. The possible identification of a specific F. nucleatum pathogen in the saliva may enablefuture targeting of this pathogen as a means of screening for, detection, treatment, follow-upor prevention of CRC"--

In the United States, hospitals annually report over 5 million cases of infectious-disease-related illnesses: clinical microbiology laboratories in these hospitals are engaged in detecting and identifying the pathogenic microorganisms in clinical specimens collected from these patients with suspected infections. Clearly, the timely and accurate detection/identification of these microbial pathogens is critical for patient treatment decisions and outcomes for millions of patients each year. Despite an appreciation that the outcome of an infectious-disease-related illness is directly related to the time required to detect and identify a microbial pathogen, clinical microbiology laboratories in the United States as well as worldwide have long been hampered by traditional culture-based assays, which may require prolonged incubation time for slowly growing microorganisms such as Mycobacterium tuberculosis. Moreover, traditional culture-based assays often require multiple steps with additional time needed for discernment of species and/or detection of antimicrobial resistance. Finally, these traditional, slow multistep culture-based assays are labor-intensive and required skilled clinical microbiologists at the bench. Over the past several decades, advanced molecular techniques in diagnostic microbiology quietly have been revolutionizing the practice of clinical microbiology in the hospital setting. Indeed, molecular diagnostic testing in general and nucleic-acid-based amplification methods in particular have been heralded as diagnostic tools for the new millennium. There is no question that the development of rapid molecular techniques for nucleic acid amplification/characterization combined with automation and user-friendly software has greatly broadened the diagnostic capabilities of the clinical microbiology laboratory. These technical advances in molecular microbiology over the first decade of the 21st Century have profoundly influenced the physical structure of clinical microbiology laboratories as well as their staffing patterns, workflow, and turnaround time. These molecular microbiology advances have also resulted in the need for a revised and updated second edition of Advanced Techniques in Diagnostic Microbiology. This second edition again provides an updated and comprehensive description of the ongoing evolution of molecular methods for the diagnosis of infectious diseases.
