Previous literature reported that people with diabetes mellitus had 70% greater risk for dementia development.Traditionally, type-2 diabetes and AD (type-3 diabetes) have been thought as independent disorders; recent literature suggests possible links that could lead to common effective modalities of treatment. Furthermore, the highly innervated pancreas shares many features with the brain at molecular levels. Metformin is recommended as the first line for the treatment of patients with type 2 diabetes mellitus due to its effectiveness, favorable effects on lipids and cardiovascular risks, and safety profile.The previous restriction of the use of metformin in patients with the moderate renal disease is loosened by the Food and Drug Organization(5,6). Thus the use of this valuable and affordable drug is expected to increase. The American Diabetes Association recommended the periodic measurement of vitamin B12 and supplementation as needed to reflect the recent evidence showing the association of long-term use of metformin and B12 deficiency.Although the role of vitamin B12 deficiency in the development of dementia is well-established, the role of metformin in B12 deficiency and dementia remain to be elucidated. Insulin resistance is considered as a primary factor for the association of diabetes mellitus and dementia. However, the role of insulin sensitizer in the prevention of dementia remains unclear.Furthermore, there is an increasing concern about the role of metformin in cognitive disorders.Thus we conducted this narrative review, in this study we reviewed the literature to assess the interaction of Alzheimer's disease, the gut, and its microbiome, and to assess the role of metformin and other drugs used for type 1&2 diabetes mellitus as possible therapies for the AD. The role of microbiota and fecal transplantation on cognitive disorders was also discussed.

Background: Dementia is highly prevalent in older age, accounts for a significant proportion of age-related disability, and is one of the most expensive disorders affecting older Australians. T2DM affects about 85% of all people with diabetes and occurs more commonly in older age. T2DM increases the risk of vascular dementia and Alzheimer's dementia (AD) although there may be substantial overlap of the two pathologies. The underlying pathways between T2DM and dementia may involve neurodegeneration, vascular disease, or both, with several common intermediary mechanisms. Aims & methods: The broad aim of this thesis was to study the disease pathways that underlie the association between T2DM and dementia. The majority of the research presented was conducted within the Cognition and Type 2 Diabetes in Older Tasmanians (CDOT) study. A further study was conducted in a second sample, derived from the United States' Alzheimer's disease Neuroimaging Initiative (ADNI).Results: The main novel results of my thesis are summarised below:1.Brain atrophy is a key mediator of T2DM-related cognitive impairment and the regional distribution of brain atrophy seen in T2DM appears similar to that seen in early AD. 2.Tissue advanced glycation is associated with brain atrophy in T2DM (and in those without T2DM) and may partially mediate the association between T2DM and brain atrophy.3.T2DM is associated with excess production of CSF phosphorylated tau, and this partially mediates the association between T2DM and reduced cortical thickness, providing the first in-vivo evidence mechanistically linking T2DM with neurodegenerative AD-type pathology.4.Retinal vascular architecture and retinopathy (subclinical markers of small cerebral vessel disease) were not associated with MRI biomarkers of T2DM-related brain disease, raising speculation about the relative importance of vascular pathways leading to brain disease in people with T2DM receiving good glycaemic and vascular risk control. Conclusions: Brain atrophy is a key mediator of diabetes-related cognitive impairment and mechanisms similar to that seen in AD may play a role in T2DM-related cognitive impairment. These findings do not exclude the possibility that cerebrovascular disease or other non-AD-type processes contribute to T2DM-related cognitive impairment. A greater understanding of the mechanisms linking T2DM and dementia may facilitate development of new avenues for treatment of dementia.

Objectives: Specific aims of the study were: (1) To develop RxDx risk index to predict dementia in patients with type 2 diabetes mellitus and hypertension (2) To compare RxDx risk index with different versions of Charlson comorbidity score (CCS) and chronic disease score (CDS) to predict dementia and (3) To compare Angiotensin Converting Enzyme (ACE) inhibitors versus Angiotensin Receptor Blockers (ARB) for the risk of dementia in patients with type 2 diabetes mellitus and hypertension. Methods: The Clinical Practice Research database was used for this retrospective longitudinal cohort study. Elderly patients (age>=65 years) diagnosed with type 2 diabetes and hypertension without prior diagnosis for dementia were included in the cohort. The Cox proportional hazard model was constructed to model time to dementia by incorporating age, gender and 31 RxDx disease conditions. Points were assigned to risk factors based on beta coefficients to obtain summary risk score. Different rick indices were compared against RxDx-Dementia risk index using c statistics, net reclassification improvement (NRI) and integrated discrimination improvement (IDI). A marginal structural model was constructed while controlling for demographic and clinical baseline variables and time-varying blood pressure variable to estimate causal effect of ACE inhibitors compared to ARB on the risk of dementia. Results: The incidence of dementia was 3.42% in patients with type 2 diabetes mellitus and hypertension. The c-statistics value for RxDx-Dementia risk index was 0.795 (95% confidence interval [CI], 0.789-0.801) and 0.806 (95% CI, 0.798-0.814). Based on the c-statisctics, NRI and IDI values the RxDx-Dementia risk index performed better compared to summary CCS, CDS scores and its combinations. The marginal structural model estimated statistically significant 39% (OR, 0.61; 95%CI, 0.50-0.77) reduction in the risk of developing dementia compared to ACE inhibitors. Conclusions: RxDx-Dementia risk index can be a useful tool to identify hypertensive diabetic patients who are at high risk of developing dementia as well as to control confounding in observational studies. ARB may offer protective effect on the risk of dementia compared to ACE inhibitors in patients with type 2 diabetes and hypertension. When there is no treatment available for dementia, prevention or delaying onset of dementia may help reduce the overall disease burden.
