Bronchopulmonary dysplasia (BPD) as a chronic lung disease affecting preterm infants has been recognized for more than 45 years. However, little progress has been made in the prevention and treatment of the disease. BPD continues to be a major morbidity affecting preterm infants. Studies in pre-adolescent children demonstrate that the abnormal lung function from BPD persists well into childhood. Infants with BPD also have an increased risk for adverse neurodevelopmental outcome. Thus BPD imposes a significant burden of adverse health outcomes in preterm infants. The major risk factors for BPD are prematurity, mechanical ventilation, exposure to noxious insults such as oxygen, infections. New research data both in basic science and clinical studies have shed light on the pathogenesis, and possible new treatment or management strategies for the future. In the proposed issue of the Clinics in Perinatology, we would like to comprehensively cover BPD both from a basic science and clinical perspective. Our attempt is to balance new information along with classic topics. The proposed authors for the issue are recognized experts in the topic area/s. We hope that the issue will be of interest to a broad array of readership.

Lung disease affects more than 600 million people worldwide. While some of these lung diseases have an obvious developmental component, there is growing appreciation that processes and pathways critical for normal lung development are also important for postnatal tissue homeostasis and are dysregulated in lung disease. This book provides an authoritative review of fetal and neonatal lung development and is designed to provide a diverse group of scientists, spanning the basic to clinical research spectrum, with the latest developments on the cellular and molecular mechanisms of normal lung development and injury-repair processes, and how they are dysregulated in disease. The book covers genetics, omics, and systems biology as well as new imaging techniques that are transforming studies of lung development. The reader will learn where the field of lung development has been, where it is presently, and where it is going in order to improve outcomes for patients with common and rare lung diseases.

In this book, you'll learn multiple new aspects of respiratory management of the newborn. For example, ventilator management of infants with unusually severe bronchopulmonary dysplasia and infants with omphalocele is discussed, as well as positioning of endotracheal tube in extremely low birth weight infants, noninvasive respiratory support, utilization of a protocol-driven respiratory management, and more. This book includes a chapter on noninvasive respiratory function monitoring during chest compression, analyzing the efficacy and quality of chest compression and exhaled carbon dioxide. It also provides an overview on new trends in the management of fetal and transitioning lungs in infants delivered prematurely. Lastly, the book includes a chapter on neonatal encephalopathy treated with hypothermia along with mechanical ventilation. The interaction of cooling with respiration and the strategies to optimize oxygenation and ventilation in asphyxiated newborns are discussed.

Neonatology is one of the areas of greatest development and evolution within pediatrics. The technoscientific advances in this area have led to an increase in the survival of premature infants who sometimes require sophisticated care. However, there is essential care that must be included in all centers that care for high-risk babies. This book includes important topics related to neonatal care grouped into four sections. In 14 chapters that address relevant issues about neonatal care, the book seeks to contribute to the clinical work of the health teams of neonatal units. Specialists in the field of neonatology from different countries have developed these chapters and through them they hope to share part of their experience.

As outlined by Dr. V. Donoghue in her preface, recent advances in the therapeutic manage ment of neonates with low gestational age have resulted in a significantly high er survival rate and better outcome for these newborns. Lung and heart malformations or acquired diseases represent a major part of the life threatening conditions in this group of critically ill patients, and radiological imaging is one of the main tools to define the appropriate therapeutic approach. This book not only provides an excellent update on the embryological and anatomical aspects of neonatal ehest conditions but also offers a unique and comprehensive overview of our current knowledge and of recent progress in imaging techniques of the neonatal ehest. Dr. Donoghue has been successful in engaging a number of outstandingly qualified international experts to contribute to this work, and I would like to congratulate her on the excellent coordination and editing of this book. I am confident that this outstanding volume will meet with great interest not only from general as well as specialised paediatric radiologists but also from neonatologists and paediatricians. I hope it will enjoy the same success as many previous volumes in this senes. Leuven ALBERT 1. BAERT Preface In the past two deeades there have been extraordinary advanees in the treatment of eritically ill neonates, resulting in improvements in their survival and a signifieant deerease in morbid ity. Neonatal sepsis still remains a significant cause of morbidity and mortality in the newborn, particularly in preterm, low birth weight infants. Despite advances in neonatal care, overall case-fatality rates from sepsis may be as high as 50%. Clinical signs of bacterial infection are vague and non-specific, and up to now there exists no easily available, reliable marker of infection despite a large bulk of studies focussing on inflammatory indices in neonatology. Every neonatologist is faced with the uncertainty of under- or over- diagnosing bacterial infection. In this book three topics will be discussed: clinical presentation including a general approach to sepsis neonatorum and two distinct diagnoses pneumonia and osteomyelitis diagnostic approaches including C-reactive protein and the immature myeloid information, and prevention and treatment of bacterial infection with immunoglobulins.

Lung disease is a major indication for the admittance of the neonate to a specialist intensive care unit, and is a particularly common complication in the pre-term baby where the lungs are insufficiently developed at birth and easily damaged by early treatments. As a consequence, this is an area of intensive international research activity. In this comprehensive update of the well-received first edition, leading researchers from all over the world have been invited to contribute in their specialist areas. The book continues to provide detailed coverage of the pathogenesis, clinical and laboratory features and management of lung disorders in the neonate, with increased emphasis on the underlying immunology and major additions to the sections on respiratory support, chronic lung disease and abnormalities in lung growth and development to reflect the changes that have occurred in these areas since the previous edition appeared in 1995. Providing an unrivalled up-to-date statement on the problems that are faced in the neonatal intensive care unit on a daily basis, this is an invaluable addition to the bookshelves of neonatologists and other personnel involved in the care of critically ill babies.
